Where you'll be working
This registered nursing position is based at Auburn Community Health Centre and requires providing care to clients in their homes and community nursing clinics within the Western Sydney Local Health District. Occasional afternoon shifts, weekends and public holidays are required, worked out of the Blacktown Community Health Centre. Approx. 1‑2 afternoon shifts and 1‑2 weekend shifts per 4‑week roster period.
What You'll Be Doing
Provide comprehensive nursing care to clients with chronic and complex health needs, working independently and collaboratively to support optimal health outcomes.
* Conduct holistic nursing assessments and develop, implement and evaluate care plans
* Coordinate care and deliver evidence‑based clinical interventions in the community
* Educate and support clients, carers and families to promote self‑management
* Maintain accurate, timely clinical documentation and ensure safe handover practices
* Contribute to continuous improvement, clinical reviews and professional development

Essential Criteria
* Registration: Current Registered Nurse division 1 AHPRA (Australian Health Practitioner Regulation Agency)

* Relevant clinical experience in speciality
* Demonstrated ability to organise and prioritise work
* Demonstrated effective communication skills both written and verbal
* Effective interpersonal skills showing initiative and leadership
* Ability to work as a member of a multidisciplinary team
* Commitment to continuing professional development
* Demonstrated ability to utilise information technology within a variety of applications and willingness to undertake training
* Current drivers licence valid within Australia and willingness to use for work purposes
