Key Responsibilities:
* Provide high-quality nursing care to patients in a general practice setting
* Assist GPs with clinical tasks such as immunisations, wound care, chronic disease management, and health assessments
* Care coordination: contribute to the development and implementation of care plans to support patient health needs
* Patient education: educate patients on preventive care, treatment plans, and overall well-being
* Patient records: maintain accurate and up-to-date electronic health records to ensure seamless coordination of care
About You:
* You hold a current registration with AHPRA as a Registered or Enrolled Nurse
* Experienced in General Practice: bring experience in a primary care setting and have a strong understanding of chronic disease management and clinical assessments
* Great communicator: possess excellent communication skills and enjoy building rapport with patients and team members
* Team player with independence: can work autonomously and as part of a multidisciplinary team, managing patient care with confidence
