Job Title: Clinical Case Manager
This is a Clinical (RN) case management position that involves managing the care of complex clients. The role requires strong clinical expertise and excellent communication skills.
The successful candidate will work closely with a team of nurses and support staff to provide comprehensive health assessments, manage client conditions, and coordinate supports across multidisciplinary teams.
The role involves a mix of in-office work and community visits, training the support team, and assisting with onboarding new clients.
1. Provide clinical oversight and mentoring for the community team.
2. Manage complex client cases and coordinate supports across multidisciplinary teams.
3. Undertake comprehensive health assessments and manage client conditions.
4. Advocate for clients and maintain strong relationships.
Requirements
1. Qualified Registered Nurse with 2-5 years experience.
2. Previous case management and/or supervisory experience is desirable.
3. Ability to work collaboratively in a team.
4. Highly developed verbal and written communication skills.
5. Confidence in managing time effectively.
