About The Role
Join our dedicated and compassionate team as a District Nurse, where you'll play a vital role in helping consumers achieve their health goals. As part of a multidisciplinary team, you'll provide holistic, consumer-centred care that makes a meaningful difference in the community.
This role involves delivering a diverse range of services through programs such as the Commonwealth Home Support Programme (CHSP), Home Care Packages (HCP), Post-Acute Care (PAC), Hospital in the Home (HITH), Home and Community Care Program (HACCPYP), Department of Veteran Affairs (DVA), the National Disability Insurance Scheme (NDIS), and private brokerage contracts.
Key Responsibilities
1. Provision and Coordination of Care: Deliver high-quality nursing care tailored to individual consumer needs.
2. Collaborative and Therapeutic Practice: Work closely with multidisciplinary teams to ensure integrated and effective care.
3. Professional Practice: Uphold professional standards and maintain current knowledge in community nursing practices.
4. Critical Thinking and Analysis: Assess, plan, and evaluate care using evidence-based practices and critical thinking skills.
Skills & Experience
1. Registered Nurse with current AHPRA registration and a minimum of three years of post-graduate experience.
2. Proven experience in a community, hospital, or acute care setting, with advanced skills in areas such as wound care, continence care, and diabetes management.
3. Strong assessment and care planning skills, with a focus on wellness and reablement principles.
4. A valid Australian Driver’s Licence.

This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024 - 2028. The classification is District Nurse Level 1 (YF4) to District Nurse Level 2 (YU1), ranging from $50.68 - $52.83 per hour.
