Support Worker (Home Care) - South East Region

* Based in Perth Metro South East region
* Part-time role, $32.75 (Level 2) or $33.81 (Level 3) per hour + Salary Packaging options and Superannuation

Hours: Five days - Monday to Friday (9.00am - 2.00pm)
Our Home Care services (CHSP, VHC, and HCP) empower older people and veterans to live safely while maintaining as much independence as possible within their own homes. You will provide individualised care tailored to each client's and their families' needs.
Primary responsibilities:
* Meal assistance, including shopping and meal preparation
* Transport and accompaniment, including shopping trips and appointments
* Social support & activities, including outings and social events
* Personal care, including assistance with grooming, hygiene, bathing, and dressing
* Support for health conditions, including skin care, medication reminders, and diabetes management
* Assistance with mobility
* Domestic assistance, including cleaning and laundry
What you will need:
* A kind, caring, and respectful approach to service provision
* Certificate III in Aged Care / Individual Support (or similar) or equivalent experience
* Self-motivated with the ability to work effectively both in a team and independently with minimal supervision
* Ability to work with individuals from diverse social, financial, and cultural backgrounds
* Strong communication and interpersonal skills
* Current driver's license and reliable car
* Current national police check (not older than 6 months)
* Prior experience in a similar community/home care role (desirable) or in Disability/Residential care
