New Aboriginal Family Support Worker role at Monash Health’s one-stop Health & Wellbeing Hub

About Infant Child Family Health and Wellbeing Hub
Monash Health is an appointed lead provider for the new Infant, Child Family Health and Wellbeing Hub (Hub), designed as a one-stop service to provide therapy across all aspects of an infant's or child’s life from 0-11 years. The Monash Health Hub has funding for community health, mental health, and family support services to enable mental health consultations, assessments, and treatments to infants, children and families in the South-eastern region of Victoria. The Monash Health Hub will sit alongside Early in Life Mental Health Service (ELMHS) - Monash Childrens and the Child and Family Program - Monash Health Community.
Children and their families will be able to access a range of specialist medical and allied health services, including paediatricians, occupational therapists, psychologists and speech pathologists to support their needs and as well as build on existing services and capacity.
The first if its kind, the Monash Health Hub in consultation with local communities, people with lived experience local service providers, including Aboriginal Controlled Community Organisations, will provide Aboriginal culturally appropriate support to Aboriginal families and their kin.
The Monash Health Hub will be centrally located at Cranbourne ,with spoke sites across the catchment of Dandenong, Casey, and Cardinia and will be part of a newly co-designed service with a Transdisciplinary/ Multidisciplinary service.
Aboriginal Family Support Worker
As the Aboriginal Family Support Worker, you are dedicated to helping Aboriginal children receive child focused family care through the new Infant Child Family Health and Wellbeing Hub with a holistic and culturally appropriate journey that includes consideration of the social determinants of health which impact the Aboriginal Community. This role involves assisting and supporting clients and their families to navigate the health system and working closely with multi-disciplinary teams in the care of Aboriginal children and their families. This new position will work in any clinical area at any or all campuses of Monash Health as and when required. Such a requirement may be a temporary arrangement, on an ad hoc basis, or a permanent change.
Part Time (.5 eft) permanent position at Cranbourne with travel to spoke sites, as required.

About You
As the Aboriginal Family Support and community development worker, you will have previous experience with supporting paediatric Aboriginal clients to provide a holistic and culturally appropriate journey through the Monash Health Hub .
To ensure your success in this role, you will have the following demonstrated skills, experience and knowledge:
1. Relevant Paediatric qualifications to demonstrate your ability to perform role.
2. A relevant case-management qualification or Children’s services qualification.
3. Identify as an Aboriginal and/or Torres Strait Islander person and demonstrate acceptance by their community and actively participate in Aboriginal/Torres Strait Islander cultural activities
4. Understanding of Victorian Aboriginal communities, and ability to build connections with Aboriginal children, families, and care givers.
5. Experience advocating and supporting the needs of marginalized and/or underrepresented communities within mainstream systems.
6. Dedicated to safe best practice and quality care
7. Committed to delivering patient centred care and a patient advocate
8. Enthusiastic, compassionate able to work collaboratively across multidisciplinary team
9. Current Drivers Licence
