About the RoleWe are looking for a passionate and skilled professional with a strong background in disability support and coordination to join us our team on a part time basis.The Support Coordinator plays a crucial role in assisting individuals with complex needs to achieve their goals of living independently and establishing valued social roles within their community. This position requires a deep understanding the NDIS framework and a commitment to upholding dignity and rights of those receiving support.
Key ResponsibilitiesSupport individuals to understand and implement their NDIS plans by promoting independent choice, control and decision makingEnsure supports are coordinated in a way that maximises positive outcomes, value for money and plan effectivenessEnsure complex and high-risk cases are managed proactively, with early identification and mitigation of risksDevelop and maintain professional and effective relationships with other service providers and stakeholdersConduct intake and assessments for new referrals identifying support needs, managing risks in line with policies and proceduresContribute to a positive, ethical and accountable organisational culture About YouYou will bring:2-5 years experience working in the disability or community services sectorPassionate and values-driven to provide high quality support, striving for positive outcomes for individualsStrong understanding of the NDIS and service systemExperience working with complex or high-risk individualsHold a minimum Certificate IV in disability service, aged care or community servicesExcellent written and verbal communication skillsA commitment to ethical, person-centred practiceThis is an office-based role in our Gympie office, with engagement in community, and flexibility upon negotiation.
