As a growing organisation, we are seeking a dedicated and organised Care Coordinator (Full-Time) to join our team. This role is ideal for someone who thrives in a fast-paced environment, enjoys problem-solving, and is passionate about improving the lives of people with disabilities.
As a Care Coordinator, you will be responsible for overseeing the day-to-day coordination of individual support services. You will ensure that client needs are being met through the effective scheduling of support staff, monitoring of care delivery, and communication with clients, families, and stakeholders.
Key Responsibilities
* Coordinate and manage the delivery of in-home and community-based support services
* Develop and maintain individualised care plans based on client needs and goals
* Schedule staff according to care requirements, availability, and preferences
* Liaise with clients, families, support workers, and other professionals to ensure high-quality care
* Monitor the quality and consistency of support being delivered
* Provide leadership and support to support workers
* Respond promptly to incidents, complaints, and service delivery concerns
* Ensure compliance with NDIS Practice Standards and Olympia Projects' policies
Qualifications & Requirements
* Previous experience in care coordination, rostering, or case management in the disability or aged care sector
* Strong understanding of the NDIS, person-centred support, and compliance obligations
* Excellent organisational and problem-solving skills
* Outstanding communication and interpersonal skills
* Ability to work autonomously and as part of a team
* Relevant qualifications in Community Services, Disability, Aged Care, or a related field (desirable)
* Current Driver’s Licence and reliable transport
* Current NDIS Worker Screening Check, Working with Children Check, and First Aid Certificate
