Clinical Case Manager Opportunity


We are seeking a part-time Clinical Case Manager to join our Adult Community Mental Health team. In this dynamic role, you will conduct mental health assessments, develop treatment plans, and provide support to individuals across various life stages.


Key responsibilities include:



* Conducting comprehensive mental health assessments to identify individual needs and develop effective treatment plans

* Providing case management services to support individuals in achieving their recovery goals

* Collaborating with emergency services and community agencies to ensure seamless care coordination

* Developing and maintaining relationships with clients, families, and community partners to promote positive outcomes

About the Role
