The Salary range is currently $51.75 to $53.84 per hour. Hourly rate based on experience.

Location : Fair Haven Aged Care is located in a semi-rural part of Hervey Bay, on 40 beautiful acres offering cool breezes, walking paths and abundant birdlife. The vibrant beach side town has everything you need with alfresco cafes, shops, parklands, picnic areas and local medical services only a short drive away.

What You’ll Do

As Care Coordinator, you’ll lead the delivery of high-quality clinical care for residents with complex needs, while working a hands-on roster that includes PM shifts on weekdays and AM shifts on weekends. This structure ensures strong clinical oversight during key after-hours periods and weekends.

In this pivotal leadership role, you will:

o Oversee resident assessments and develop tailored care plans
o Coordinate clinical documentation and AN-ACC compliance reporting
o Provide direct, on-the-floor clinical support and leadership
o Mentor and support a team of RNs, care staff, and volunteers
o Liaise with external providers to ensure seamless care delivery
o Monitor and enhance clinical programs and quality indicators
o Foster a culture of safety, wellbeing, and continuous improvement

About You

As a Care Coordinator, you’ll play a pivotal role in leading clinical care delivery for residents with complex needs. You’ll oversee assessments, care planning, and compliance, while mentoring a team of Registered Nurses, care staff, and volunteers. Your leadership will ensure our residents receive holistic, person-centred care aligned with our Positive Wellbeing Model.

This is a key leadership position reporting to the Service Manager, with direct responsibility for clinical staff and indirect influence across the broader care team.

Requirements:

o AHPRA Registration as a Registered Nurse (RN) with a current Practicing Certificate
o Minimum 3 years’ post-graduate experience in aged care or acute clinical settings
o Proven leadership and workforce management skills
o Strong clinical assessment, care planning, and documentation capabilities
o Intermediate computer literacy (Word, Excel, Outlook, PowerPoint)
o A commitment to quality, safety, and continuous improvement
o Current National Police Check – or willingness to acquire
o Current First Aid & CPR certification – or willingness to acquire
o A Valid driver’s licence

Desirable:

o Postgraduate qualifications in Gerontology, Dementia, or Aged Care
o Experience with mobile clinical documentation systems
