What You'll Be Doing
The Senior Clinician Community Health will be responsible for providing support to patients with late stage chronic and complex health conditions including:
Care planning and advance care planning
Palliative care needs assessments and collaborative treatment care plans
Referrals to relevant internal and external health and community support providers
Education/support to other clinicians in supporting complex patient needs
Selection Criteria
Current Registration as an Occupational Therapist with AHPRA or eligibility for accreditation as an Accredited Exercise Physiologist with ESSA
Extensive knowledge and experience working with consumers with chronic and complex illnesses, including advance care planning discussions
Experience in care planning and case management of patients in the community with chronic and complex illnesses
Understanding of specialist palliative care, a palliative approach and community health services
High level interpersonal and communication skills, including written, verbal and computer skills, and ability to work in a multidisciplinary team
Clinical leadership capabilities with the ability to provide consultancy, education and support to individuals, families, allied health and nursing staff, and other health professionals
Current unrestricted drivers' licence and ability to drive/travel as required (may involve long distances and overnight stays)

Occupational Assessment, Screening and Vaccination
This is a Category A position.
