**End-of-Life Care Specialist**

The End-of-Life Care Specialist will provide advanced clinical assessment skills and management experience to support high-quality patient-centered care by promoting shared decision-making practices through:
* Staff education, training, and mentoring on end-of-life care.
* Facilitated advance care planning discussions with patients to document future care preferences.
* Comprehensive and collaborative end-of-life care and planning for patients requesting access to voluntary assisted dying as an end-of-life option.
This position works closely with the Advisor, Shared Decision Making, the Director of Medical Services, and the Chief Medical Officer to ensure the provision of safe and effective person-centered care.
This is a permanent part-time role (72 hours per fortnight) located at Monash Medical Centre, however working across all Monash Health sites is required.
**Key Responsibilities**
Provide clinical and organisational support for the implementation of the Voluntary Assisted Dying Act (2017).
Conduct comprehensive psychosocial assessments and coordinate access to VAD eligibility assessments for those patients requesting access to voluntary assisted dying.
Develop and implement models of care that improve consumer healthcare experiences.
Work collaboratively with consumers, health care professionals, and service providers to deliver comprehensive person-centered end-of-life care.
Support the ongoing development of a person-centered voluntary assisted dying model of care for the organisation.
**Requirements**
Normally have at least 10 years' post-qualifying experience.
Completion of a postgraduate qualification relevant to the position requirements.
Understanding of the Medical treatment planning and Decisions Act (2016) and the Voluntary Assisted Dying Act (2017).
Current Victorian Drivers Licence essential.
Australian working rights.
For Allied Health: must comply with the Position Description for a Grade 4 - relevant to your Occupation Qualifications.
Current general registration with the Australian Health Practitioner Regulation Agency (AHPRA) for relevant occupation.
For Social Worker: Bachelor of Social Work or Masters Social Work Qualifying OR Masters of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW).
For Nursing: must also comply with the Position Description for a Clinical Nurse Consultant C (attached), and be Registered with Nursing and Midwifery Board of Australia.
Postgraduate qualification or working towards the same.
