Lead advanced, patient‐centred care for older people in Toowoomba. As a Nurse Practitioner, you'll combine clinical expertise with autonomy to assess, diagnose and treat complex conditions, while working closely with a multidisciplinary team to improve outcomes for a vulnerable population and shape innovative models of care.

As Nurse Practitioner – Frail Older Persons, you'll play a vital role in improving care for older people with complex needs. From leading advanced clinical decision‐making to shaping integrated care pathways, you'll work alongside a collaborative team to deliver meaningful, patient‐centred outcomes across the service.

  • Delivering advanced assessment, diagnosis and treatment that improve outcomes for frail older patients
  • Working collaboratively with multidisciplinary teams to coordinate care and support informed patient decisions
  • Leading and influencing best practice through clinical governance, quality improvement and innovation
  • Mentoring and guiding colleagues while strengthening systems that support safe, effective care

Here's What Will Help You Thrive In This Role
  • Endorsement (or eligibility) as a Nurse Practitioner with current registration
  • Master's degree qualification relevant to Nurse Practitioner practice
  • Advanced clinical expertise in managing acute and chronic conditions in older people
  • Strong leadership, collaboration and critical thinking skills in multidisciplinary settings

You'll stand out from the crowd if you have experience working in geriatric or community‐based models of care and/or a passion for driving service innovation and improvement.
