The Role
We are currently seeking Occupational Therapists to join our friendly and supportive Home and Community Care team. You'll work in our Transition Care Program (TCP) at Juniper Hayloft in Martin, supporting older adults to regain their independence after a hospital stay. This role sits within Juniper's Allied Health Team and forms part of our wider multidisciplinary team. You'll play a vital role in supporting our reablement philosophy, helping older adults regain their independence and improve their functional abilities. We have an exciting position for an Occupational Therapist for Juniper TCP Hayloft, with 4-5 days available per week. Specific working hours will be negotiated directly with the successful candidate.

Key Responsibilities
Conduct client assessments to determine capabilities, goals, and support needs
Deliver tailored, goal-oriented therapy to support independence
Prescribe home modifications, aids and equipment as required to promote independence
Play a pivotal role in discharge planning in collaboration with the multidisciplinary team
Delegate and work with Allied Health Assistants where appropriate
Provide guidance to Allied Health Assistants where required
Communicate effectively with clients, families, and multidisciplinary teams
Collaborate across the organization to ensure high-quality care delivery
Be part of multidisciplinary team meetings
Contribute to quality improvement and best practice initiatives
Work across Home Care Package (HCP), Commonwealth Home Support Program (CHSP), Support at Home (SAH) and TCP funding streams
Key Requirements
Current registration as an Occupational Therapist with AHPRA (essential)
Valid Australian driver's licence and reliable vehicle (essential)
Previous experience in aged care, community health, or rehabilitation settings (desirable)
Demonstrated ability to conduct functional assessments and develop person-centred therapy plans
Experience working with clients with dementia, chronic illness, or mobility limitations (highly regarded)
Knowledge of aged care quality standards, funding models (HCP, CHSP, TCP), and clinical documentation
Ability to work independently and as part of a multidisciplinary team
Effective time management and organisational skills to manage a varied caseload
Strong interpersonal and communication skills to engage respectfully with older clients, families, and stakeholders
Commitment to continuous improvement, innovation, and person-centred care
