About the Position
As a Social Care Professional, you will be working in an office-based setting to provide high-quality person-centred care to individuals facing severe mental health challenges.
This role involves building strong therapeutic relationships with clients, coordinating with healthcare professionals, and supporting clients throughout medical and social appointments.
Key Responsibilities
* Conduct thorough clinical assessments to develop personalized care plans that meet the unique needs of each client.
* Collaborate effectively with General Practitioners (GPs), psychiatrists, psychologists, and other allied health professionals to deliver comprehensive care.
* Support clients in advocating for themselves during medical and social appointments, ensuring their voices are heard and their needs are met.
* Engage with family members and carers as appropriate to provide holistic support to clients.
Requirements
* A degree in Social Work or a related field is essential for this role.
* At least 2-5 years of experience in community mental health or social support roles is required.
* A solid understanding of psychosocial support principles and trauma-informed care is necessary to succeed in this position.
* Excellent communication and stakeholder engagement skills are critical for effective collaboration and relationship-building.
