In this role you will be providing advanced nursing assessment, diagnosis, care and intervention to patients and families within the scope of practice of a NP/TNP Palliative Aged Care Supportive Service (PACSS).
You will also provide high level clinical nursing, leadership, strategic governance and management in establishing a model of care for those patients and their families in their transition to end of life care.

Where you'll be working Mona Vale Hospital What You'll Be Doing The Nurse Practitioner/Transitional Nurse Practitioner (NP/TNP) for the Palliative Aged Care Supportive Service (PACSS) will provide advanced palliative care support to older people residing in Residential Aged Care Facilities (RACFs) across the Mona Vale region.
Working in partnership with the NSLHD Palliative Care service and the BRACE program, the NP/TNP will deliver expert clinical assessments and care planning.
This role will also collaborate closely with key stakeholders including Hammond Care Palliative Care, Northern Sydney Home Nursing Service (NSHNS), and General Practitioners supported by the Northern Sydney PHN, ensuring coordinated, person-centred palliative care for aged care residents.
The practice of the NP/TNP is in accordance with current best evidence and all relevant NSW Health and NSLHD policies, procedures, and guidelines, including compliance with the NSW Health Policy Directive Nurse/Midwife Practitioners NSW PD2012_026.
Operating within a defined scope determined by the Nursing and Midwifery Board of Australia's NP Standards for Practice and the Australian Nursing Code of Ethics, the role functions at an advanced practice level, independently managing a range of palliative and end-of-life care options that best meet the needs of patients, carers, and families.
Skills The NP/TNP extends nursing practice by enabling timely patient reviews and effectively managing palliative and end-of-life care, addressing identified service gaps.
As a palliative care expert with strong aged care assessment skills, the NP/TNP supports the education and professional development of nursing staff, allied health, patients, carers, and families.
A key role is to facilitate the transition of aged care residents to end-of-life care while coordinating with relevant NSLHD services.
Scope of Work: Providing autonomous, evidence-based clinical care for patients in Residential Aged Care Facilities (RACFs) and as inpatients at Northern Beaches Hospital Conducting comprehensive assessments to identify physical, emotional, and spiritual needs, and implementing advanced, holistic management plans Prescribing evidence-based therapeutic interventions tailored to patient needs Leading advance care planning and optimizing symptom management across physical, psychological, social, and spiritual domains Referring complex cases beyond scope to BRACE Geriatricians, Palliative Medicine specialists, and other healthcare teams Acting as a liaison between community and inpatient services to ensure seamless care coordination.

Registered Authorised NP with APHRA that has completed the Tertiary requirements including a Master's Degree (Nurse Practitioner) OR Registered Nurse willing to enrol in a Tertiary Master's Program for Nurse Practitioner.
Minimum of 5,000 hours of advanced nursing/midwifery practice during the last six years appropriate to the specialty area (or working towards within the next 3 years).
Ability to independently assess, investigate and implement appropriate treatment for patients within advanced nursing practice guidelines.
Demonstrated clinical competency in advanced practice management in Supportive and Palliative Care and specific interest in aged care.
Demonstrated ability to work autonomously and have effective communication and negotiation skills.
Demonstrated ability to work independently within the wider multidisciplinary team.
