WHAT ARE WE OFFERING?
We are looking for a skilled and compassionate Geriatric Clinical Nurse to join our dedicated team within the Barossa Hills Fleurieu Local Health Network. In this role, you will provide expert clinical nursing care and support the development and delivery of effective Geriatric Outpatient services. You will play a vital role in coordinating holistic, person-centred care for older adults across multidisciplinary teams, helping to improve health outcomes for vulnerable populations.

HOW WILL YOU CONTRIBUTE?
* Provide proficient, person-centred clinical nursing care and individual case management to older adults in a defined clinical area.
* Support the coordination, triage, and prioritisation of patients referred to geriatric outpatient clinics.
* Collaborate effectively with multidisciplinary teams and other aged care services to ensure holistic care and service integration.
* Lead and support clinical education, mentoring less experienced staff and students, and contribute to continuous quality improvement initiatives.
