About your role: * Provide care to acutely unwell clients in their own homes. * Responsible for assessment and support of Community patient's health literacy and escalation of care as appropriate. * Responsible for a specific client population while seeking support and direction from Community Clinical (Intake) Nurses, Doctors, and other non-registered nursing personnel. * Delegation of care to assistants in nursing and healthcare workers. * Driving your own or Queensland health vehicle.
