Job Overview
We are seeking a compassionate and dynamic Care Coordinator to join our management team and assist in the day-to-day planning and running of our bespoke home care services. The Care Coordinator plays an integral role in ensuring that our customers receive excellent continuity of care and that our Care Team is well-organised and supported. Full training shall be provided with regular ongoing coaching and mentoring from the senior leadership team to ensure you are confident in the role. The contract is for a minimum of 12 months (maternity cover) with the potential for an extension and/or permanent position.
Duties
* Completing the rostering of customer care visits and scheduling employee shifts.
* Managing changes to the care roster in response to customer demand and staff absence.
* Collaborating with and assisting the management team with various business administration tasks e.g. compliance audits, HR records, care plan updates, and customer reviews.
* Developing and maintaining professional relationships with our team, customers, and external partners / healthcare professionals.
* Handling telephone, text, and email enquiries with professionalism and courtesy. This includes helping with onboarding staff, customer acquisition, and liaising with other agencies.
* Providing support and assistance to the care team with their rota, training, and holiday.
* Managing the on-call out-of-hours service on a rotational basis - one weekday per week, and one weekend per month.
* Covering care calls in emergencies.
Skills & Experience
* 1 year+ experience in a Care Coordinator role or similar (desirable).
* Knowledge and awareness of CQC regulatory compliance (desirable).
* A strong commitment to enhancing the quality of life for vulnerable adults.
* Excellent organisational skills with attention to detail.
* Excellent communication and interpersonal abilities.
* Adaptable, flexible, proactive, and someone who shows initiative.
* Enjoys finding workable solutions and solving day-to-day challenges.
* Computer literacy (essential).
* A valid driver's licence and vehicle access (essential).
* Level 3 NVQ / Apprenticeship in Health & Social Care or equivalent (desirable).
