We are seeking a skilled Care Services Manager to oversee a portfolio of clients in South Grafton. The successful candidate will develop and manage care plans tailored to individual needs, ensuring exceptional service delivery.
Key Responsibilities:
* Administer tasks such as budgeting, monitoring financial transactions;
* Liaise with staff to deliver high-quality services;
Requirements:
* Demonstrated experience in home care packaging, CHSP, DVA, case management, assessment, care planning, and service coordination;
* CURRENT CPR Certificate;
* Solid understanding of client-centered, community-based care service delivery and associated quality standards;
* Ability to promote services and liaise with external providers;
* CURRENT driver's license with own fully insured vehicle;
* Eligible for not-for-profit salary packaging;
