Community Based Support (CBS) is a not-for-profit Tasmanian organisation delivering aged care and disability services to enable people to live safely in their homes and enjoy access to the community as they wish. Service Delivery Coordinator – Programs
The Service Delivery Coordinator is responsible for thecoordination of CBS services and supports delivered to clients receiving funding from a range of Commonwealth and State Government sources. These may include Home Care Packages (HCP), Commonwealth Home Support Program (CHSP), Home and Community Care (HACC); Veterans Home Care (VHC); Hospital Patient Sitting (Royal Hobart Hospital - RHH) and the National Disability Insurance Scheme (NDIS) – in addition to self-funded clients.
The Coordinator ensures clients receive a high standard of quality care which is delivered in line with CBS and external regulatory and legislative obligations. The Coordinator also supervises a teamof staff who provide supports to clients in home care, community, hospital and centre-based settings.
Core Responsibilities
Coordinate the effective and efficient provision of CBS-delivered supports and services to clients within budget and according to legislation and funding body guidelines
Undertake home visits with clients to ensure their needs and requirements continue to be met
Manage client complaints and concerns with care and discretion, seeking timely and mutually successful resolutions
Manage a short-term service program, in partnership with The Royal Hobart Hospital, to help discharged patients recover and receive care in their own homes
Respond to and manage client and staff hazards and incidents, to ensure the safety and wellbeing of CBS clients and staff
Ensure client files, records and plans are maintained in an appropriate and timely manner
Lead a team of field staff including provision of ongoing feedback and support, lead team meetings, performance oversight and annual performance reviews
Assist in the recruitment of Field Workers including leading interviews, induction and onboarding.
Participate in after-hours on-call duties as per the rotating roster shared across the service delivery team
Ongoing support, supervision, and opportunities for professional development

About you:

You bring minimum 2 years' experience within a similar position in a large multi-faceted organisation (desirable)
Your experience reflects your knowledge and understanding of various disabilities, working with individuals who live with disability and their carers / support networks
What you must have:
Relevant qualifications and experience in the Community Services sector (preferably including Aged Care services)
Current Working with Vulnerable People Registration (or ability to obtain); and
A National Police Check that is no older than 3 months (or ability to obtain)
