Were seeking a Clinical Team Leader (Registered Nurse) to join our dedicated team at Just Better Care Mid North Coast.

The role
As a Clinical Team Leader you will :
Lead supervise and support a team of nurses and Community Support Professionals fostering a collaborative team environment.
Oversee the development implementation and review of individual care and health plans ensuring alignment with best practice and participant goals
Conduct comprehensive clinical assessments and respond proactively to changing health needs and risks.
Coach mentor and train staff in clinical skills and competencies fostering a culture of continuous improvement.
Collaborate with participants families and health professionals to deliver integrated respectful and outcome-focused services.
Monitor risks respond to changing participant needs and ensure compliance with aged care disability and health standards
Contribute to organisational performance through incident management quality improvement initiatives and strategic service delivery.
Duties and responsibilities may include :
Working with or under the guidance of medical practitioners and Allied Health professionals
Medication Administration and Complex Clinical Care eg.
palliative care bowel care / suppositories catheter care stoma care oxygen and more
Monitoring and communicating on the customers wellbeing and reporting on progress of customers
Ensuring that all services are provided in a caring and respectful way in accordance with Just Better Cares policy and procedures legislative requirements and program specific guidelines (i.e. DVA Community Nursing)
Providing training coaching and guidance to Community Support Professionals Enrolled Nurses and Support Staff / Case Managers as required
Conducting skills assessments of Community Support Professionals performing complex care services as required
Demonstrating a commitment to best practice and evidence of continued professional development
Participating in staff meetings and attending training opportunities when required
What you will need :
Current Registered Nurse registration with Australian Health Practitioner Regulation Agency (AHPRA)
Evidence of continued professional development in accordance with the AHPRA standards
Current Criminal Record Check and Working with Children Check
Current CPR / First Aid Certificate
Current Australian Drivers Licence
Excellent leadership written and verbal communication skills
Computer literacy
Commitment to person-centered trauma-aware and culturally safe care
Experience in the supervision of staff members
Knowledge of continuous quality improvement principles
Excellent people problem solving and conflict management skills
Ability to work as part of a team and maintain good working relationships
Desirable :
Previous experience in the community care or health / aged care sector
Understanding of DVA Community Nursing guidelines or previous community nursing experience
Cert IV in Workplace Training & Assessment
