As a Disability Support Worker, you'll be part of a dedicated team delivering high-quality, person-centred support within a 24/7 program based in Hawthorn. Your role will focus on assisting a participant to maintain their daily routines, achieve their goals, and participate in activities that are important to them.
No two days will look the same. From supporting daily living activities and personal care to encouraging community participation and promoting independence, you'll play an important role in helping create a safe, positive and empowering environment.

Key Responsibilities
* Provide person-centred support within a 24/7 care environment
* Assist with daily living activities including personal care and routine support
* Support community access and social participation
* Deliver health-related supports in line with care plans (including PEG feeding)
* Promote independence, choice and wellbeing at all times
* Maintain accurate documentation and communication with the broader care team
* Follow support plans and report any changes or concerns appropriately
About You
To be successful in this role, you will have:
* A valid P2 or Open driver's licence and reliable vehicle
* NDIS Worker Screening Check and Working with Children Check
* Experience supporting participants with complex support needs
* Confidence delivering personal care and health-related supports
* PEG feeding experience
* Strong communication and interpersonal skills
* A calm, reliable and person-centred approach
