Our Opportunity: We are looking for a talented Registered Nurse to join our Remedy family. Working as part of our multidisciplinary healthcare team, you will coordinate and support the delivery of home-based health care to clients across Australia.
This great part-time opportunity will be worked remotely on Saturdays and Sundays from 10am - 2pm on a 12 month fixed term contract.
The day to day
Coordinating and managing acute Hospital Care at Home programs for clients discharging early from hospital to home-based care
Supporting and communicating with clients remotely to achieve their personal and clinical health goals within their home environment
Implementing, monitoring and modifying in-home care plans to meet client needs
Loading new cases into the system
Performing ad-hoc administrative tasks as needed
Managing clinical case escalations and variations to expected clinical progress
Managing relationships with multiple stakeholders including hospital referrers, surgeons/specialists, service providers and funders
About You
Qualification as a Registered Nurse with current AHPRA registration
Experience working in a multidisciplinary collaborative team
Excellent knowledge of hospital discharge planning
Tech savvy - this role requires working across multiple systems
Ability to work in a dynamic environment with tight timeframes and competing priorities
Excellent clinical reasoning and problem-solving skills
Excellent verbal and written communications
Strong administration and time management skills
Passionate about health and committed to making a real difference in the community
