The HMHB team are an integral part of the Child and Family program within Monash Health Community.
The team is comprised of Social Workers, Community Development Workers and a Nurse/Midwife that are dedicated to improving the health and wellbeing outcomes for pregnant women, their babies and families.
We are committed to providing women with high quality care, support, education and advocacy throughout their pregnancy and following birth, which embrace family centred, strengths based and evidence-based practice, within a Social Model of Health.

**A Day in the Life**:
As a Monash Health employee you will have the ability to maintain a high level of communication with clients, colleagues and other internal and external care providers, enabling you to deliver excellent care.
As a member of the team you are integral to the Monash Health Social Work service delivery and consistently demonstrate our core values. You will be part of a flexible workforce with personalised support.
You will provide a comprehensive range of care and services to pregnant women and their families experiencing complex vulnerability. Support is provided through an outreach model of care, supporting women within the hospital setting and the community including their homes, outpatient clinics, and other community venues.
You will provide excellent Psychosocial and risk assessment, assisting women to develop goals and access appropriate community supports to address risk factors and improve the health and wellbeing outcomes for themselves and their babies.
Work across a variety of clinical specialisms providing evidence based clinical Social work to women who are pregnant and until approximately 6-8 weeks post birth.
Deliver a high standard of written reports and documentation within agreed timelines.

**What You Bring**:
- Qualifications and relevant paediatric experience as a Community Development worker thats worked with women and families who provides a strong focus on client centred care
