Job Description
Epworth Rehabilitation is seeking to appoint a dynamic and experienced Grade 2 Social Worker to provide comprehensive social work services at Epworth Camberwell. This is a permanent part-time position of 22.8 hours per week. The successful applicant will provide excellence in patient care to patients and their families predominantly in the area of aged care. Clinical responsibilities include comprehensive psychosocial assessments, family work, outpatient work, group work, discharge planning and involvement in student supervision. The role will be primarily based at Epworth Camberwell with the potential to work across other clinical areas at Hawthorn and Richmond.
You will work in an energetic and cohesive clinical team that strives for excellence in patient care. The Social Work Department offers a high level of clinical support and supervision, as well as a strong commitment to professional development and quality projects.
Responsibilities
* Undertaking comprehensive psychosocial assessments and complex discharge planning in the aged care space
* Striving for patient and family inclusive practice in all stages of care
* Engaging positively with all other colleagues to achieve best clinical outcomes
* Supervising students
* Demonstrating flexibility and adaptability in a dynamic clinical environment
Qualifications
* Bachelor or Master of Social Work
* Eligibility for membership of the AASW
* A minimum of three years of relevant social work experience in a hospital, rehabilitation or other healthcare setting
* Clinical experience and/or knowledge in the area of aged care would be highly regarded
* Well-developed expertise in assessment, intervention and discharge planning
* Inherent knowledge of aged care systems, compensable systems (such as TAC and WorkCover), the NDIS, Centrelink, VCAT and other community agencies
