The position involves shift work, requiring you to be available to work flexible shifts that may include a combination of weekdays and weekends.
Key Responsibilities
* Collaborate with customers to develop individualized support plans that incorporate their identified needs and goals.
* Provide support to customers to improve their independence by assisting with daily tasks, transport, social activities, medication, and personal care.
* Offer psychosocial support to customers to explore their practical and complex needs.
* Maintain customer records up-to-date using online systems.
* Develop and maintain professional relationships with customers, families, caregivers, and stakeholders.
* Seek innovative ways to enhance the customer experience.

In addition, you'll need:
* Technical literacy with Microsoft Office and reporting systems.
* Minimum Cert IV in Mental Health or Disability or equivalent industry experience.
* Availability to work across a rotating roster.
* Willingness to use a personal mobile phone for work purposes.
* Valid NSW Drivers C class license with ability to drive large vehicles.
* Willingness to obtain necessary checks prior to commencement.
