About This Role
Carer Gateway supports people who provide care for someone living with disability, mental illness, chronic illness, terminal illness or age-related frailty. At Wellways, we support carers to access the services, information and connections they need. As a Carer Services Navigator, you will work directly with carers to understand their goals, connect them with supports, and help them build stronger support networks.
This is a community-based role where you'll work with carers in a range of settings including homes, community locations and health services.
Your work will focus on two key areas:
Supporting carers to navigate services
* Work with carers to understand their needs and goals
* Develop and review individual carer support plans
* Connect carers with services and supports in their community
* Coordinate referrals and support the delivery of services
Facilitating peer support
* Support carers to connect and learn from each other through peer groups
* Facilitate structured sessions using peer support principles and the CHIME recovery framework (Connectedness, Hope, Identity, Meaning and Empowerment)
You will also collaborate with health and community professionals such as hospital discharge planners, social workers and aged care teams to support referrals for carers.
What You'll Bring
* Strong communication and relationship-building skills
* Experience supporting people in community or health settings
* The ability to work independently while managing referrals and service coordination
* A passion for supporting carers and strengthening communities
Pre-employment Compliance Requirements
* Certificate IV or above in a support discipline and/or Equivalent Community sector experience
* Ability to travel to rural and remote regions (Roma/Goondiwindi)
* Current valid Driver's Licence
* Satisfactory National Police Records Check (within 12 months)
* WWCC / QLD Blue Card
* NDIS Workers Screening check / QLD Yellow Card
* Appropriate IT Skills
* Right to Work within Australia
* NDIS Worker Orientation Module Certificate
