As a Youth Lead Practitioner, you will play a vital role in supporting adolescents and young adults aged 9-25 with disabilities. This is an exciting opportunity to make a real impact in the lives of young people and their families.
Key Responsibilities:
* Led the interdisciplinary youth team that plans a young person's NDIS funded therapy package, acting as the main contact for adolescent and young adult participants and their families
* Utilised assessment and intervention skills to support youth participants to achieve their educational, personal, social, and working life goals
* Worked within the social model which means you'll have worked and delivered therapy in diverse settings, from homes and schools to playgrounds, workplaces and community centres
* Held meaningful, future-focused conversations with participants and their supports
* Empowered participants and their families to develop habits and routines
* Built meaningful relationships with your participants and their supports (family, carers, friends)
* Documented, and billed for, each session through our online participant management system

Requirements:
* A relevant tertiary qualification and registration
* Minimum 12 months experience working with youth aged 9-25
* A positive and flexible approach to work
* Strong communication skills and be a team player
* Commitment to personal and professional growth
* Demonstrated experience working with people with a disability
* Willingness to work to billable targets
* A valid Australian driver's license
* NDIS Workers Screening Check
* Working with Children's Check
