About The Role
The Clinical Nurse Specialist Grade 2 (CNS2) Hospital Avoidance coordinates and delivers high quality nursing care to patients/clients in accordance with the Nursing and Midwifery Board of Australia's National Standards of Practice for Registered Nurses, criteria for CNS2 as outlined in the Public Hospital Nurses and Midwives (State) Award and the Policies and Procedures of the Ministry of Health (MoH) and Northern NSW Local Health District (NNSWLHD).
Playing a pivotal role in enhancing patient care by facilitating timely, appropriate, and coordinated healthcare services within the community, inclusive of Hospital In The Home (HITH) and the Aged Care Outreach Service (ACOS).
