Position Overview
We are currently seeking a motivated and compassionate Social worker to join our multidisciplinary team at Seymour Health. The position includes supporting Ambulatory Care, Acute Ward, Hdx, HARP (Hospital Admission Rick Program) and Barabill House Aged Care Facility. This position is a permanant 0.5 EFT position (rostered days flexible). For further information on the role, please refer to the Position Description attached.
Qualifications
Ability to independently provide clinical reasoning and clinical competencies, including expertise in the use of relevant assessment tools, treatment methods, and case management skills
Experience working with people experiencing complex needs, health conditions, including trauma, substance use, or family violence
Experience in person centered care and advocacy
Experience with discharge planning and service availability
Knowledge around Aged Care process and NDIS
Ability to respond timely to referrals
Sound understanding of Advance Care Planning aims and underlying principles

Seymour Health is a leading small rural health service. At our site in central Seymour, over 350 staff deliver health and allied services. Our services include a 30-bed public hospital that provides public and private Acute medical and surgical, renal dialysis and day procedures. Seymour Health's breadth of services extends to post-acute care, palliative care, sub-acute ambulatory service, oral health services, district nursing and community services. Barrabill House is a 40-bed residential aged care service that provides highly rated residential aged care. Seymour Health is conveniently located an hour north of Melbourne via the M31 Hume Freeway and V/Line train service.
Employment Requirements
All employment with Seymour Health is subject to and conditional upon a satisfactory Police Check and Victorian "Employee" Working with Children's Check and NDIS Worker screening check. All employees of SH must comply with the health service's vaccination requirements procedure (as amended from time to time) together with any relevant State Government and Department directives.
