Job Role:
A Community-Based Support Practitioner is required to deliver high-quality, person-centred services throughout the Peel, Rockingham and South Metro regions.
Key Responsibilities:
* Deliver short-term and long-term supports to individuals and their families across a range of disability-related needs.
* Work in partnership with support networks and carers to develop tailored plans that meet the unique needs of each individual.
* Conduct home visits and community-based assessments to identify and address support needs.
Work Environment:
The role will be based in clinics located in Bertram and Mandurah, with regular travel across these regions as required.

Qualifications and Skills:
* Relevant Bachelor's degree qualification in a related field.
* Working with children check.
* Valid West Australian drivers' licence.
* Ability to reside in and hold full working rights in Australia.
* Experience in the allied health sector working with NDIS participants and knowledge of the NDIS framework.
