Working both independently and collaboratively, you will deliver high-quality social work interventions that enhance client outcomes and strengthen support networks. Essential Criteria Qualifications: Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW) Selection Criteria Demonstrated post graduate clinical experience working with clients with chronic and complex Demonstrated clinical experience or understanding of the needs of clients accessing the health care system. Demonstrated clinical experience or understanding of the needs of clients with chronic and complex health needs. Well-developed written, verbal and non-verbal communication skills, effective time management and ability to prioritise workload. Able to work as an effective team member as well as work independently, including flexibility and adaptability. Good computer skills, including experience using Cerner eMR or equivalent. Demonstrated understanding and application of quality improvement principles. Drivers license valid within Australia and willingness to use for work purposes.
