The Neighborhood of Somerset is seeking CNAs to join their team The CNA reports directly to the Director of Nursing.
Shift Details
Monday - Friday; every other weekend required.
Purpose
CNA - Certified Nursing Assistant
The Certified Nursing Assistant is responsible for providing the highest quality of care and services to residents and their families in our assisted living and memory care neighborhoods. This includes providing hands-on care, physical and emotional support as outlined in each resident's Personalized Service Plan (PSP), while maintaining a safe, comfortable, home-like environment. The Resident Care Associate demonstrates all elements of wellness, adhering to federal, state, and local standards and regulations, as well as Phoenix policies, to promote high-quality care.
Principle Duties and Responsibilities
* Participate in developing and updating Personalized Service Plans; attend service plan review meetings with families as requested.
* Follow the hydration schedule established for residents during the shift.
* Complete resident meal attendance sheets; ensure residents who do not dine in the dining room are provided room service or accounted for.
* Know and comply with all resident rights.
* Initiate, assign, and assist with activities as appropriate.
* Check assigned residents daily for cleanliness, appropriate clothing, continence issues, and changes in condition.
* Use proper sanitary procedures and universal precautions.
* Check on assigned resident rooms daily for cleanliness and take appropriate action; ensure high-risk rooms are double-checked.
* Assist with laundry rooms and common areas, including bathrooms, checking for cleanliness and hazards, and take appropriate action (e.g., contacting housekeeping, cleaning, or securing chemicals).
* Complete all ADLs for assigned residents according to their PSP.
* Respond promptly to security system alerts and resident call bells, taking appropriate action including resetting call bells.
Education/Experience/Licensure
CNA - Certified Nursing Assistant
High school education preferred and may be required by the state. SC State CNA Licensure. Previous experience working with elders or disabled individuals is preferred. Must be at least 18 years old to perform personal care tasks. Knowledge and experience in assisted living, dementia care, or skilled nursing are preferred. Ability to work weekends, evenings, and flexible hours to meet peak service times.
