Job Description

This role is a critical part of our team's effort to provide comprehensive support to individuals with complex needs. The successful candidate will work closely with participants who have mental health conditions, Acquired/Traumatic Brain Injury (A/TBI), or disabilities resulting from workplace or traffic incidents.

The primary focus of this position is to empower participants to manage daily tasks independently, with a strong emphasis on empowerment over direct intervention. To achieve this, the ideal candidate will employ effective communication and interpersonal skills, coupled with a deep understanding of case management and referral processes.

* Key Responsibilities:
* Community outreach and telehealth support, case management, and following established behavioral support plans
* Regular communication with care teams, linking participants to a wide range of community and clinical services
* Developing professional relationships with individuals, their families, and other stakeholders in the community
* Managing referrals, performing risk assessments, providing accurate and timely case notes and reports


Required Skills and Qualifications

* Essential Attributes:
* Effective and efficient rapport-building skills
* Strong listening skills, adaptable communication style, and a desire to learn more
* Contribution to a positive team environment and ability to work autonomously
* Familiarity with technical terminology in a clinical setting
* Previous experience in mental health or outreach support, preferably supporting individuals with complex needs; ABI/AOD/mental illness/physical disability/dual diagnosis
* Desirable Attributes:
* Qualifications in Mental Health, Social Science, or other relevant fields (or actively pursuing studies)
* Valid driver's license and willingness to use personal vehicle for work-related travel within a specified radius
