Deliver Culturally Safe, Strengths-Based Care to Our Communities
Join the HOMES Team within Metro South Health's Aboriginal and Torres Strait Islander Health Services as a Clinical Nurse (Case Manager). This permanent full-time position is based at the Southern Queensland Centre of Excellence, Inala, and plays a pivotal role in delivering culturally safe, holistic, and strengths-based case management for Aboriginal and Torres Strait Islander clients with complex health needs.
About This Role
Your new role:

As a Clinical Nurse – HOMES Paediatric and Adolescent Case Manager (Identified), you will provide advanced clinical leadership, case coordination, and culturally informed care to Aboriginal and Torres Strait Islander clients across Metro South Health. Working within a collaborative, multidisciplinary model, you will empower clients to lead their own health journeys, build self-determination, and improve long-term wellbeing outcomes.

You Will

* Deliver person-centred, culturally safe nursing care and case management to clients with complex needs.
* Build and maintain trusting relationships with clients, families, and communities based on mutual respect and understanding.
* Collaborate with multidisciplinary teams and community organisations to ensure continuity of care.
* Participate in home visits, outreach, and follow-up care to support accessibility and engagement.
* Lead and mentor staff in culturally responsive practice, education, and service improvement.
* Contribute to research, evaluation, and continuous improvement initiatives within the HOMES model.

This is an identified vacancy with a genuine occupational requirement for the position to be filled by a candidate indigenous to the Aboriginal or Torres Strait Islander Community. Candidates not meeting this criteria will not be considered.

About You
You are an experienced and compassionate nurse who is passionate about improving health outcomes for Aboriginal and Torres Strait Islander peoples. You demonstrate cultural insight, strong communication, and the ability to lead within a multidisciplinary environment.

You Will Demonstrate

* Current registration with the Nursing and Midwifery Board of Australia (NMBA).
* Proven experience providing culturally appropriate nursing and case management services.
* Strong understanding of Aboriginal and Torres Strait Islander social, emotional, and cultural wellbeing frameworks.
* Ability to develop, implement, and evaluate individualised care plans in collaboration with clients and families.
* Leadership and mentoring experience within a healthcare team.
