About the Role:We are seeking an experienced and compassionate Care Supervisor (Clinical) to join our client's dedicated team. This pivotal role goes beyond supervision; it is about providing clinical leadership, coordinating complex care, and ensuring every client receives exceptional, person-centred support that aligns with their life goals. You will be the key link between clients, their families, and a multidisciplinary care team, ensuring seamless, high-quality service delivery that promotes dignity, independence, and well-being.
Key Responsibilities:Clinical Leadership & Consumer-Centred Care:
* Provide clinical leadership, supervision, and delegation to a team of care staff, offering mentorship and constructive feedback to uphold the highest standards of practice.
* Coordinate a caseload of clients with complex health and social care needs, conducting comprehensive assessments, developing tailored care plans, and evaluating interventions using evidence-based and person-centred principles.
* Act as a clinical expert, observing, reporting, and proactively responding to changes in a client's condition, and adjusting care plans or escalating concerns appropriately.
* Provide direct, compassionate support and interventions, including emotional support and assistance with therapy plans for clients with specific needs such as dementia.
Coordination, Advocacy & Compliance:
* Liaise effectively with external healthcare providers, allied health professionals, and assessment organisations to coordinate services and ensure holistic management of each client's needs.
* Advocate strongly for clients' rights, including confidentiality, privacy, and supported decision-making, ensuring their voices are heard in all aspects of their care.
* Ensure strict compliance with all relevant legislation, Aged Care Quality Standards, NDIS requirements, and organisational policies, including the company's Code of Conduct.
* Lead the investigation and resolution of client incidents and feedback, participating in quality reviews and continuous improvement initiatives to enhance service delivery.
Operational Management & Safety:
* Manage client care budgets and home care agreements effectively, identifying opportunities to optimise package utilisation in line with client goals and preferences.
* Identify, assess, and document potential risks, implementing robust risk management strategies to ensure client and staff safety.
* Champion a culture of safety by adhering to and promoting all Work Health & Safety (WHS) policies, reporting hazards and near misses, and participating in safety activities.
* Maintain accurate, timely, and confidential client records within the organisation's electronic systems, ensuring all documentation meets professional and regulatory standards.
Team & Community Engagement:
* Build and maintain strong, effective relationships with clients, their families, colleagues, and the broader community.
* Foster a collaborative, inclusive, and accountable team environment that is responsive to client needs.
* Actively promote the organisation's community philosophy, events, and range of services to clients and their networks.
Skills & Experience:
* A minimum of a Diploma in Aged Care, Nursing, or a relevant Allied Health field (Bachelor's degree highly regarded).
* A minimum of one year experience in a clinical or supervisory role within aged care, community care, or a related health sector.
* In-depth understanding of Aged Care Quality Standards, Home Care Package (HCP) guidelines, and relevant compliance frameworks.
* Excellent clinical judgement, problem-solving skills, and the ability to work autonomously as a practitioner.
* Outstanding verbal and written communication skills, with the ability to adapt communication styles to diverse backgrounds.
* Strong organisational and coordination skills, with a proven ability to manage competing priorities and complex caseloads.
Mandatory Requirements:
* A current NDIS Worker Screening Check (not more than 5 years old).
* Satisfactory National Police Check with no disclosable outcomes.
* Current Driver's Licence (Class C) and reliable, insured transportation.
* Up-to-date immunisation status in accordance with public health directives and organisational policy, including annual influenza and COVID-19 vaccinations.
* A signed statutory declaration regarding any serious criminal activity committed outside of Australia.
* Current First Aid Certificate.
* Proof of professional registration (e.g., with AHPRA for Nurses) where applicable.
