About your opportunity:
Provide clinical and non-clinical home-based face to face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT). Under the Single Aged Care Assessment System, Aged Care Assessors work with clients and their representative/s to establish goals specific to the client's needs and develop appropriate strategies and actions, including referrals to aged care and non-aged care services.
Requirements:
Mandatory qualifications (Nursing or Allied Health) and proof of registration (if applicable).
This is a Vaccine Preventable Disease (VPD) risk role.
Possession of class C drivers' licence.
Travel is a requirement of this position.
Probation period may apply.
Pre-employment checks will be conducted including criminal history clearance.
