What You\'ll Be Doing
Join a dynamic team providing essential social work support to heart failure patients at Blacktown Hospital's outpatient clinic. Make a meaningful impact through compassionate, evidence-based care.
* Deliver psychosocial assessments and interventions
* Support patients with complex health and social needs
* Engage in supervision, training, and ongoing development
* Contribute to multidisciplinary care planning
* Maintain accurate records and departmental reporting

Essential Requirements
Qualifications: Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW).
Selection Criteria
* Demonstrated core competency skills in psychosocial assessments, and demonstrated understanding of issues associated with: domestic violence, child protection matters, acute and chronic illness, age related illnesses, guardianship and end of life matters.
* Demonstrated clinical experience or understanding of grief and loss counselling, crisis intervention, discharge planning and working with complex family dynamics.
* Demonstrated understanding of matters associated with working with people from Aboriginal and Torres Strait Islander background as well as people from culturally and Linguistically Diverse backgrounds.
* Well-developed time management skills, excellent written and verbal communication, including report writing skills and high level computer literacy.
* Ability to work with minimal supervision and as part of a multidisciplinary team
* Good knowledge of community resources and experience liaising with community organisations
* Commitment to quality improvement and evidence-based practice and ongoing clinical and professional learning.
* Ability to work in a team providing 7-day rotating roster and participate in the After Hours On Call Social Work Service.
