Macmillan Clinical Nurse Specialist Palliative Care

An exciting opportunityhas now arisen within the CommunitySpecialist Palliative Care Team for a Clinical Nurse Specialist in Palliative Care.

In this role, you willwork across the Barnsley neighbourhoods, providingsupportive palliative care to enhance the quality of life for patients/carerswith any life limiting illness requiring palliative care, including complexphysical, psychological & emotional needs.

The post holder willpossess extensive knowledge of palliative medication to support the complexsymptom management needs of the patient.

As Band 7 Macmillan CNSPalliative Care, you will be able to demonstrate:

ability to managea caseload of people with specialist palliative care needs

excellent workingknowledge of pain & symptom management & managing complex emotional& psychological needs

ability toprovide highly specialised advice/guidance to members of the wider health &social care team

ability to workas an effective team member

Main duties of the job

As Macmillan Clinical Nurse Specialist Palliative Care, you will:

Assess, develop &implement specialist palliative care for patients & family, monitoring& evaluating the effectiveness of that care enabling patients to bemaintained in the environment of their choice

Work closely withneighbourhood teams, primary care, & adult social care partners to ensureseamless support for individuals with complex needs

Be equipped with the knowledge, understanding, skills,attitudes & behaviours relevant to employment & act in accordance with the NMC Code Professional standards of practice

Havea key role in training, education & staff development, actively participate in regular supervision &appraisals contributingto clinical leadership facilitating a culture of learning/reflective practice

Have high level communication & interpersonalskills

Job responsibilities

JOB SUMMARY

To assess, develop and implement specialistpalliative care for patients and family of those diagnosis with cancer andother life limiting illness, monitoring and evaluating the effectiveness ofthat care enabaling patients to be maintained in the environment of theirchoice.

Provide and lead specialist education toothers to promote best practice in Palliative Care.

KEY RESULTAREAS:

Thepostholder will:

1.1Care/service/functionprovision:

Tomanage own specific geographical caseload, and day to day management ofcolleagues in absence of team members.

Togive highly specialised advice, information, guidance and support to patients,families, Neighbourhood teams, Primary Health Care Team, Allied Health CareProfessionals and Social Care Professionals.

Toprovide highly specialist clinical leadership for patients and their familiesto meet individual palliative care needs in the community, workingindependently with significant discretion.

Toautonomously assess, plan, implement and evaluate the care for individualpatients and their families to meet individual palliative care needs in thecommunity, working independently with significant discretion.

Whereit is appropriate to the service, upon successful completion and registrationof the relevant qualification with the individuals professional body and SWYPFTto function as an independent non-medical prescriber and carry out relevant CPDto maintain competence.

Todevelop effective and empathetic supportive relationships with patients andtheir families, using advanced communication skills to relate highly complex,contentious, sensitive information taking into account barriers tounderstanding and challenging communication.

Tomonitor and identify complex bereavement issues and refer to appropriateservices.

Toact as a resource in developing and signposting information for patients andstaff.

1.2 Working as part of a Team:

Toact as Palliative Care representative at primary care Multi-Disciplinary TeamMeetings.

Tobe a core member of Palliative Care Multi-Disciplinary Team meeting andneighbourhood team.

Toinitiate, design and deliver palliative care education programmes for multiprofessional groups to promote high quality palliative care.

Toact as a mentor to newly appointed clinical nurse specialists in palliativecare setting

Tocontribute to business planning for service development and strategic planningin palliative care services

Todevelop network and regional contacts enabling the sharing and promotion of newdevelopments in Palliative Care.

Toparticipate in Team responsibilities for efficient management of PalliativeCare Services, including the accepting, recording and safe keeping of donationsfrom the general public for Macmillan and Home Care Support Fund.

Toparticipate in the formulation and implementation of policies and proceduresand evaluate standards of care within palliative care services.

Todevelop and lead integrated audit systems, which influence and guide palliativecare practice in line with Department of Health Guidelines.

Toparticipate in national research projects.

Tomaintain accurate manual and computerised records to ensure continuity of careand facilitate accurate data collection for service development

1.3Managing Self:

Tocritically evaluate and disseminate existing research to promote evidence-basedpractice incorporating into SWYPFT Clinical Governance Guidelines.

Toparticipate in professional development and clinical supervisionprogrammes.

Tobe aware of and act in accordance with Trust clinical standards, guidelines,Health and Safety and work within the NMC Code of Conduct.

Tohave developed flexible self-management skills and have the ability toconcentrate where the work pattern is unpredictable.

1.4 Management Supervision and Support

To be part of the core neighbourhood team and provide leadership forpalliative/ end of life care team for the team. This includes provision ofsupervision and clinical advice and direction.

1.5 Financial Responsibility: eg, ** Use Trust resources efficiently and suggest efficiency savingsto more senior staff).

Person Specification


Special Knowledge/Skills

* Knowledge of current issues affecting provision of Palliative Care.


Experience

* Experience of working at band 6.
* Demonstrable post Registration experience including working within Palliative Care/end of life/cancer care settings.
* Advanced Communication and interpersonal skills.
* Ability to work in a multi professional team.
* Ability to work as autonomous practitioner.
* Proven planning and organisation skills.
* Evidence of ability to develop, implement, deliver, plan and evaluate teaching packages/education programmes.
* Proven analytical and leadership skills.
* Ability to utilise research methodology.
* Ability to manage fluctuating conflict situations.


Personal Attributes

* A current driving licence and access to a car during the working day is essential (reasonable adjustments will be considered for any applicants who are unable to drive due to a disability).


Physical Attributes

* Ability to undertake the duties and demands of the post. A satisfactory sickness record over the previous 2 years (subject to the need to act with fairness and equality of opportunity, particularly where the sickness is related to a disability and/or pregnancy).


Training

* Evidence of continuing professional development.


Qualifications

* 1st Level Registered Nurse (RGN).
* Educated to 1st degree level.
* Diploma or above in Specialist Palliative Care.
* Experience of mentoring/supporting learners or ability to demonstrate the skills required for effective mentoring.
* Counselling qualification.
* Community qualifications/experience.
* Independent non-medical prescribing.
* Masters degree in Palliative Care related area or working towards.
