This position is located at our Extendicare Van Daele home in Sault Ste. Marie, Ontario.
What You’ll Be Doing
1. Coordinate the completion of MDS assessments on all residents within established timelines.
2. Understand how the MDS assessment integrates with existing clinical tools and provide leadership in streamlining these processes.
3. Participate in strategic planning for program development and improvement while staying current on RAI trends.
4. Interpret reports generated by Health Zone/AH&W or CIHI and provide critical analysis for the Home.
5. Participate in planning and organizing the home RAI process integration and sustainability.
6. Provide on-site RAI MDS 2.0 education to staff as part of orientation and ongoing reviews.
7. Manage the coordination and timely completion of resident assessments, tracking forms, RAPs, and care plans.
8. Assist RAI users with correct assessment coding and support validation of coding.
9. Oversee the RAI process according to policies, regulations, and interRAI requirements.
10. Provide leadership and mentoring for education, data accuracy, and system applications, including implementing a data quality program.
11. Develop processes with management and team members to ensure data integrity, efficient collection, and utilization.
What You Bring
1. 3-5 years of experience in Long Term Care; experience with MDS is an asset.
2. Current registration with the College of Nurses of Ontario and in good standing.
3. Knowledge of InterRAI tools, MDS/RUG III classification systems.
4. Success in leading new initiatives.
5. Excellent oral and written communication skills.
6. Experience with adult education strategies and facilitation.
7. Current RAI-MDS and AIS certifications.
8. Ability to maintain confidentiality.
9. Strong analytical skills and experience in data trending and analysis.
10. Knowledge of Point Click Care and RAI-MDS.
11. Ability to develop comprehensive care plans.
12. Flexibility in working hours and shifts.
13. Ability to collaborate with interdisciplinary teams and stakeholders.
14. Strong problem-solving skills.
15. Proficient with computers.
16. Must provide a current Vulnerable Sector Check and Criminal Background Check or be willing to obtain one.
