About the Opportunity
We are seeking a Care Manager to lead our team and ensure high standards of care are maintained in Mildura.
The Role
You will support the General Manager in managing and leading the delivery of person-centred care to residents, ensuring ongoing assessment, care planning, and evaluation, as well as clinical governance.
* Oversee daily clinical operations to ensure residents receive best possible care.
* Support and lead the care home clinical team, including coaching, mentoring, staff allocations, and delegation.
* Review clinical incidents, complaints, and changes, providing solutions to reduce risks.
* Lead compliance with AN-ACC assessments.
* Manage person-centred care planning and evaluation for residents, including case conferencing with families and carers.
* Foster a strong team environment.
* Prepare and conduct clinical governance audits.
* Provide insights and innovative solutions for continuous clinical improvement.
* Oversee medication management in accordance with resident needs and legislative requirements.
* Liaise with allied health, medical practitioners, and other services to provide optimal resident care.
What You Need
* Bachelor of Nursing with current APHRA registration essential.
* Postgraduate qualification in aged care highly regarded.
* Strong leadership, communication, and interpersonal skills.
* A person-centred care approach.
* Possession of passion for nurturing a strong team environment.
* Ingrained empathy for residents.
* Proven track record in delivering quality clinical care within an aged care or acute setting.
* Excellent clinical assessment and triaging skills, as well as strong care planning capabilities.
* Demonstrated experience in fostering a reporting culture balanced with effective investigative skills.
* Understanding of Resident Rights, Aged Care Accreditation Standards, and Outcomes.
