About The Opportunity
Are you a skilled and passionate nurse leader with expertise in palliative care?

South Western Sydney Local Health District (SWSLHD) is seeking a dedicated Clinical Nurse Consultant (CNC) to provide expert clinical advice and leadership within the specialty of palliative care.

This pivotal role supports community patients, carers, and healthcare professionals by developing, implementing, and evaluating tailored care management plans that improve outcomes for palliative care patients.

The CNC will work collaboratively across hospitals and multidisciplinary teams to coordinate care and ensure best-practice, evidence-based treatment is delivered.

This position is based in Rosemeadow CHC and will involve travel across the South West Sydney region.

What You'll Be Doing
The Clinical Nurse Consultant (CNC) is an expert Palliative Care practitioner who, through leadership facilitates evidence based practice. The Palliative Care CNC will promote a service which fosters clinical excellence, research and professional leadership. The Palliative Care CNC contributes to quality health care by collaborating with the health care providers and consumer groups, developing clinical practice standards and evaluating their outcomes. The Palliative Care CNC is responsible for providing services which facilitate effective management of the care in the given specialty.

Where You'll Be Working
SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.

These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at-risk communities.

With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.

How To Apply

* Current Authority to Practice as a Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA). At least 5 years full time equivalent post registration experience, with at least 3 years full time experience in the palliative care field and an approved post graduate nursing qualifications in palliative care or such other qualifications or experience deemed appropriate by the organisation.
* Demonstrated ability to apply expert knowledge to provide safe compassionate quality care that is culturally responsive.
* Demonstrated ability to effectively communicate and engage with patients/consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care need.
* Demonstrated understanding and participation in risk management, work health and safety and quality improvement in the workplace.
* Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
