Pay starting at $17.94 an hour

This position requires someone who has had a Driver's License for at least 1 year (post‐permit) and a clean driving record that meets Sevita's insurability policy.

Summary

* Position type: Non‐exempt, full‐time or part‐time, hourly role.
* Scope of role: Responsible for providing direct support within a single program or unit; typically supporting 4‐6 individuals in residential settings, or up to 10 individuals in periodic programs.
* Key responsibilities:
o Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
o Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.

Essential Job Functions

Services and Supports

* Direct Support: Provides direct support or assistance in accordance with individual service or program plans; assists with socialization and behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living.
* Records: Accurately documents progress and activity; reviews records and logs; maintains confidentiality.
* Management of Individuals' Assets: Makes cost‐effective purchases within individual budgets and according to their choice/preference.
* Relationships: Maintains healthy and professional relationships with individuals, families, guardians, case managers; greets visitors; implements Sevita customer service standards.
* Reporting: Reports alleged abuse or neglect per internal and external standards; reports medical, behavioral, and other incidents per Sevita policy and external regulations.
* Rights: Maintains confidentiality; respects rights of persons served; practices universal precautions; assists individuals in exercising their rights.

Health Care

* Appointments: May accompany individuals to medical appointments; relays orders and information to providers.
* Medications: If assigned, accurately administers or supports self‐administration; documents delivery; reports errors; maintains security of controlled medications.
* General Health Care: Monitors health; documents concerns; communicates with nurse/supervisor; calls 911 in emergencies; follows health care directives.
* Medical Supplies and Equipment: Follows procedures; reports malfunction.
* Meal Preparation: Prepares or supports individuals to prepare meals per menus; considers special diets; monitors mealtime and implements dining plans.

Employment Responsibilities

* Training: Attends orientation and ongoing training; participates in monthly staff meetings.
* Employee Scheduling: Works scheduled hours; arrives on time; stays until replacement arrives; notifies supervisor when unable to work; may fill in shifts without unapproved overtime.
* Teamwork: Works with others, values contributions, is courteous, communicates openly, listens, shows respect.
* Workplace Safety: Compiles and maintains safe work environment; complies with safety policies; reports hazards; participates in safety training.

Maintenance

* Vehicles: Transport individuals; drive safely; use safety equipment; report accidents.
* Housekeeping: Performs daily/seasonal housekeeping; maintains safe environment.
* Maintenance and Repair: Monitors safety; makes minor repairs; may perform yard maintenance; reports equipment/facility repairs.
* Safety: Checks water temperature; participates in drills; protects individuals during emergencies.

Other

* Performs other duties and activities as required.
* MPA = Mobile Punch Authorized for timekeeping.

Supervisory Responsibilities

* None

Minimum Knowledge and Skills required for the Job

* High school diploma or equivalent preferred.
* Six months of experience in human services preferred.
* Must be 18 years of age.
* Working knowledge of computers.

Certificates, Licenses, and Registrations

* Valid driver's license in good standing.
* Car registration and vehicle insurance if providing transportation.
* All state‐required training(s) and certification(s) completed in mandated timeframes.
* Maintain valid driver's license; meet insurance requirements; be approved to drive; comply with MVR policies.

Work Environment

* Works at the program location and may accompany individuals into the community.

Physical Requirements

* Heavy work: Exerting up to 100 pounds occasionally, up to 50 pounds frequently, up to 20 pounds constantly.

* Employees must cooperate with licensee and Department of Human Services staff in inspections, inquiries, investigations.
* Must have a valid driver's license in good standing in the State of New Jersey; if living in another state, must have a valid license in that state.
* At minimum, a high school diploma/equivalent is required.
* All employees hired after 6/15/2018 submit to drug testing prior to employment and randomly thereafter.
* Employees must submit to criminal background checks and be determined not disqualified by DHS prior to working; must have name checked against CRUI and CARI.
* Employees must successfully complete and demonstrate proficiency in all required trainings.
* Employees shall be at least 18 years of age.
