Austin Health is one of four health service 'hubs' in Victoria which operate radiotherapy services. Austin Health operates two radiation oncology centres in Victoria under the State Governments hub and spoke model. The metro hub is based at the Olivia Newton-John Cancer Wellness & Research Centre (ONJ Centre) in Heidelberg and the Ballarat Austin Radiation Oncology Centre (BAROC) is housed in the Ballarat Regional Integrated Cancer Centre (BRICC). Both centres provide multi-disciplinary care including both nursing and allied health services, innovative and progressive approaches to treatment underpinned by the guiding principle of patient centred care and wellness for all patients.

The multi-disciplinary teams at both sites are committed to excellence in practice and care. Staff undertake a range of professional development activities, are involved in research trials and liaise and consult with a wide range of specialists and health care professionals to ensure the care offered is at the leading edge of technology.

About the role

This role is to lead the implementation and ongoing delivery of the nurse-led follow-up clinic for prostate cancer patients post-radiotherapy, in alignment with the endorsed model of care for BAROC, and as directed by the BAROC RO Campus Director. You will work from the BAROC campus.

A regular day will see you:

* Liaising with the multi-disciplinary team to identify patients eligible for transition to a nurse led survivorship pathway.
* Consulting with men with prostate cancer and their carers, in the acute post definitive radiotherapy period - providing education, assessing and managing toxicities and symptoms.
* Coordinating and monitoring follow up investigations and escalation to the medical team as per the established pathway.
* Assisting men with prostate cancer and their carers post definitive radiotherapy to transition from an acute model to survivorship care.

About you

You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You'll bring energy and drive to your work, have a keen interest in developing your skillset, and an enthusiasm for quality improvement

To be successful in this role, you'll bring with you:

* Advanced clinical assessment skills

* Experience managing patients with cancer, ideally men with prostate cancer

* Strong communication skills and confidence to be a patient advocate within the multi-disciplinary team
