Job Title: Aboriginal Health Worker/Practitioner
About the Role:
As a key member of our multidisciplinary team, you will work to provide flexible, holistic, and culturally sensitive healthcare services to Aboriginal and/or Torres Strait Islander people. You will focus on prevention and management of chronic disease for Aboriginal clients and support them throughout their journey in custody back into community.
Responsibilities:
- Provide face-to-face patient care using a cultural lens
- Work collaboratively with nursing staff, Integrated Care Service, health professionals, Corrective Services NSW, Aboriginal Community Controlled Health Organisations, and other relevant health and community groups
- Identify patients of concern and escalate to senior health professionals or practitioners as needed
Requirements:
- Hold a Certificate IV in Aboriginal Primary Health Care (Practice) and be registered with the Australian Health Practitioner Regulation Agency (AHPRA)
- Eligible to drive in NSW and able to travel as required
- Willingness to obtain Cert IV in Aboriginal Primary Health Care (Practice) if holding a minimum Certificate III qualification
Working Arrangements:
- 38 hours per week
- Permanent Full Time position
- Salary range: $69,410.00 - $90,357.00 per annum plus super and leave loading

Mid North Coast Correctional Centre, Metropolitan Remand and Reception Centre, Silverwater Women's Correctional Center, Sydney and regional locations
