The role will be based at these sites amid a major redevelopment of Ryde Hospital, which will transform local healthcare services.
What You'll Be Doing
Provision of care coordination for older people who present with complex mental health issues across two sites. This may include physical co-morbidity, psycho-social challenges and addiction of substance use. The successful applicant will provide education and guidance to internal and external partners, including Residential Aged Care Facility staff.

Candidate Requirements
Registered Nurse with a minimum of 5 years full-time postgraduate experience in a mental health specialty. Must hold an approved postgraduate qualification in mental health drug and alcohol or similar.
Demonstrated experience working with consumers and carers presenting with complex mental health needs and/or behaviours of concern; ability to assess, plan, implement and evaluate interventions and care plans.
Extensive experience conducting comprehensive mental health assessments and developing, implementing and evaluating consumer-focused care plans based on evidence-based practice.
Additional Information
This is a NSW Health Category A position requiring immunisation and screening for certain diseases. Applicants must complete the OASV Undertaking/Declaration Form and TB Assessment Tool during recruitment and provide vaccination evidence.
