What you'll be doing
* The Manager Integrated Care is responsible for the strategic, operational management and leadership of the Far West LHD Integrated Care Program.
* Overall program development, implementation, management, reporting, monitoring and evaluation will be undertaken to improve health outcomes, patient experience of care and better co-ordination of services across the continuum of care.
* Partner with a wide range of stakeholders to redesign health services across sectors to support the implementation of contemporary integrated models of care in Far West LHD.

About Broken Hill

How to apply
To be considered for this position, please ensure you address the selection criteria in the application as thoroughly as possible:
* Senior leadership experience in a substantial and complex health environment.
* Strong record of achievement at a senior level in the health care sector.
* Comprehensive understanding and experience in the delivery of primary health care in a rural setting.
* Demonstrated ability to lead a major program of work including high level project management, change management and delivery of Programs within deadlines and within budget.
* Excellent leadership, communication and interpersonal communication skills; including the ability to negotiate with a range of stakeholders to achieve organisational reform.
* Excellent organisational skills and the ability to manage competing priorities in a high volume work area.
* Demonstrated application of knowledge and expertise in the interface between acute, sub-acute, primary, community and integrated health service delivery across a diverse range of care settings.
* Current licence to drive in NSW with the willingness and ability to drive for work purposes.
