We are seeking a compassionate and skilled Support Worker to provide high-quality, person-led support for a male participant living with dementia in a Supported Independent Living (SIL) home. The ideal candidate has proven experience supporting people with behaviours of concern, understands positive behaviour support (PBS), and can foster a safe, predictable, and dignified living environment.
We are actively recruiting specifically for the participant however other shifts within the organisation are available and may be offered to the right candidate
Key Responsibilities
* Provide daily living support: personal care, medication assistance, meal preparation, domestic tasks, and routine planning.
* Implement behaviour support strategies in line with the participant's Behaviour Support Plan (BSP), with a focus on proactive, least-restrictive, trauma‑informed approaches.
* Support communication, orientation, and independence for a person living with dementia (use of prompts, visual cues, and calm de-escalation).
* Monitor wellbeing, behaviour data, bowel and fluid intake charts, sleep patterns, and incident reporting as required.
* Maintain a safe environment, including environmental prompts, door-alert awareness, and proactive risk management.
* Promote community access, meaningful activities, and positive engagement aligned to the participant's goals.
* Work effectively within a rostered team, handover clearly, and collaborate with family, clinicians, and the provider's leadership.
* Uphold NDIS Quality & Safeguards standards, privacy, dignity, and human rights at all times.
* Willing to assist with personal care, domestic assistance and community access
Essential Criteria
* Experience: Minimum 12 months supporting people with behaviours of concern (NDIS, residential care, mental health, or dementia care).
* Dementia capability: Ability to use validation, redirection, structured routines, and sensory regulation strategies.
* PBS knowledge: Familiarity with Positive Behaviour Support, data recording, and following a Behaviour Support Plan; understanding of reportable incidents and restrictive practices (least restrictive, time-limited, authorised where applicable).
* Qualifications:
* Certificate III (or higher) in Individual Support/Disability/Aged Care/Mental Health (or equivalent experience).
* Current First Aid & CPR.
* Current Manual Handling competency.
* NDIS Worker Screening Check (or eligibility to obtain) and DHS checks as required in SA.
* Current Driver's Licence and access to a safe, reliable vehicle (desirable if community access required).
* Medication assistance (trained and competent), infection prevention & control, safe use of assistive technology (hoists, slide sheets) where applicable.
* Communication: Excellent verbal and written skills; confident with digital note-taking, incident reports, and shift handovers.
* Availability: Capacity to work a rotating roster including active nights, weekends, and public holidays.
Desirable
* Training or exposure to MAPA, PART, PMVA, TCI, or similar safe crisis management frameworks.
* Experience in SIL or shared living environments.
* Understanding of NDIS Code of Conduct, Risk & Safeguards, and rights-based practice.
* Knowledge of best-practice dementia supports (orientation aids, environmental adjustments, meaningful routines).
