Job Summary
The role of Case Manager is pivotal in supporting young parents aged 13-19yrs (residential) and 13-24yrs (outreach), and their children, to achieve their case plan goals.
Key Responsibilities
* Deliver individualized support to young parents with complex needs and behaviours to develop independent parenting and living skills through integrated case management, behaviour support, and direct parenting and emotional support.
* Foster a supportive environment for families, including the development of life and parenting skills.
* Ensure effective collaboration with staff and clients from diverse backgrounds, upholding respect and humility.
Requirements
* Possess proven experience in casework/case management, encompassing holistic assessment of client needs and case plan development.
* A relevant degree in social work, social welfare, or psychology is highly desirable for this position.
* Mandatory requirements include a Working with Children Check and a Current Australian Drivers Licence.
